PE RULES

Perc Rule:

The PERC rule is for patients where the diagnosis of PE is being considered but are felt to be low-risk.

Criteria:

Age ≥50

HR ≥100

O₂ sat on room air <95%

Unilateral leg swelling

Hemoptysis

Recent surgery or trauma

Surgery or trauma ≤4 weeks ago 

Prior PE or DVT

Hormone use

Oral contraceptives


If all of these are negative then the patient has a less than 2% risk of PE and may not need a study.

Wells’ Criteria for Pulmonary Embolism
Criteria:

Clinical signs and symptoms of DVT

PE is #1 diagnosis OR equally likely

Heart rate > 100

Immobilization at least 3 days OR surgery in the previous 4 weeks

Previous, objectively diagnosed PE or DVT

Hemoptysis

Malignancy w/ treatment within 6 months 

If all negative then <1.6% chance of PE
