TELEHEALTH

MISSION:

To develop a fellowship that will create the future leaders of this emerging field of emergency care. Specifically, a major focus of this fellowship is in how to build and grow a telehealth program. Fellows will be exposed to the key components of a telehealth program including:

  • Administration
  • Quality & Patient Satisfaction
  • Innovation
  • Clinical Excellence in multiple fields of telehealth
  • Call Center / Workflows & Processes
  • Research
  • Leadership
  • Education

ORGANIZATION:

The fellowship will an academic offering within the Division of PreHospital & Disaster Medicine in the Department of Emergency Medicine at Zucker/Hofstra School of Medicine. Research will be within that division which rolls up to the Emergency Medicine Research Consortium.

1. Administration

Fellows will participate in weekly administrative meetings as well as subcommittee meetings (operations, etc.). Fellows will be expected to learn about the following:

  • Telehealth billing / coding and revenue cycle
  • Telehealth credentialing and onboarding
  • Telehealth charting, billing and documentation
  • Basic budgeting, scheduling and management

Fellows will be expected to assist in the creation of policies relevant to Emergency Telehealth across its many use cases. The administrative component will be supported by the Northwell Emergency Medicine Service Line Administrative team, which manages the 17 emergency departments within Northwell.

2. Quality & Patient Satisfaction

Fellows will participate in bi-weekly NETS staff meetings and be expected to develop one analytic quality project over the course of their fellowship. The fellow will also be included in a monthly quality/process improvement committee.

3. Innovation

Fellows will be expected to have one innovation project during their fellowship. Fellows with a technological background will be encouraged to explore the technology that creates an effective Emergency Telehealth experience. Innovation projects can include any of the following:

  • Development of new programs with health system resources (lab, imaging, EMS, etc.)
  • Improvement of existing processes by developing automations / Application Programming Interface integrations
  • Development of dashboards or visualizations
  • Improvement of the Emergency Telehealth discharge and follow-up process

EXPERIENCES:

1. Clinical

  • As a strong background in Emergency Medicine is foundational to effective Emergency Telehealth, fellows will provide 20 hours of clinical coverage a week where they will function as an attending.
  • Fellows will cover two telehealth shifts approximately 24 hours a week. During their telehealth shifts, they will have a faculty member to assist them with clinical and technical scenarios that require more experience. Some of these shifts will be in person at Northwell’s Center for Emergency Medical Services.
  • Telehealth shifts can have periods of available time which are ideal for working on research and administrative projects.

2. Operational

  • Developing guidelines and protocols as the Emergency Telehealth program grows into acute, unscheduled care within multiple specialties and settings
  • Explore how to enhance telehealth capabilities to give the patient optimal experience during their telehealth encounter
  • A committee member for the Emergency Telehealth Quality/Process Improvement Committee
  • Quarterly financial meetings
  • Biweekly data analytics presentations
  • Upcoming year budget creation and analysis of previous year budget

4. Educational

  • Fellows will have the opportunity to enroll and complete advanced online telehealth training, including leadership courses offered by outside institutions
  • Fellows will be expected to present didactic presentations to the three emergency medicine residency programs at Northwell/Hofstra

                     i. Leading the resident telehealth track for those residents interested in telehealth careers

  • Fellows will also be charged with overseeing resident telemedicine elective rotations, telehealth simulation sessions with residents and Medical Student telemedicine rotations
  • Professional opportunities for the Fellow

                     i. Billing, CMS reimbursement rules
                     ii. Clinical Leadership Development Program
                     iii. Clinical research/grant writing

  • Fellows will be tasked with organizing Emergency Medicine resident and senior medical student elective rotations in Emergency Telehealth
  • Charged with leading periodic journal club for the Emergency Telehealth faculty

5. Research

  • Fellows will be an integral part of the Emergency Telehealth research group, and publications are anticipated.
  • Participating in development of telehealth research, which will be guided by the Emergency Medicine Research Consortium, including study design, implementation, grant writing, and manuscript submission
  • Leading periodic journal club for the Emergency Telehealth faculty
