This little article emphasizes many of the points that Dr. Ramnarine hammered home today. Remember that sickle cell whacks up your kidneys such that Cr is not a reliable indicator of GFR using standard formulas (http://www.ncbi.nlm.nih.gov/pubmed/23894560). Hence a Cr of 1.2 is highly abnormal in sickle cell.

II. CHEST X-RAY AND PNEUMONIA – http://www.ultrasoundoftheweek.com/uotw-34/

I saw Jesse G. get fired up about this at conference: chest X-ray is a piss poor test for pneumonia. What are the numbers you ask? It may only be 40-50% sensitive for the diagnosis (http://www.ncbi.nlm.nih.gov/pubmed/?term=Am+J+Emerg+Med.+2013+Feb%3B31(2)%3A401-5). What about ultrasound you ask? The numbers are better with a sensitivity of > 90% (http://www.ncbi.nlm.nih.gov/pubmed/24758612). So let’s all put our Koenig caps on and sono some lungs because to find all these pneumonias you gotta be an expert with ultrasound.
